The staff member completes part 1 ((questions 1 – 5) and part 2 (ratings only and returns the entire form to their supervisor, electronically. Please work with your supervisor regarding deadlines, scheduling your formal review meeting, and any specific items he/she would like you to address in your self-assessment. Supervisors are responsible for meeting with their staff, providing feedback and allowing for a discussion.
